Residential and Personal Emergency Identification Cards

Emergency Identification Cards give first responders the information they need to help you faster and more safely. Click here to request your cards. (Print-it-yourself versions are at the bottom of this page)

Residential Card Information: SJSO offers a Residential Emergency Card for placement on the refrigerator. This card, which is bright orange in color, can be a huge help in communicating vital information to emergency workers who may have to respond to your home. There are spaces for vital information, such as:

  • Name and Address

  • Emergency Contact Information

  • Doctor’s Name and Phone Number

  • Health Care/Insurance Information

  • Allergies to Medications

  • Major Illnesses

  • Current Medications and Dosages

Personal Emergency Card Information: This card, which can be carried either in your wallet or in the case provided, would be beneficial in the event of an emergency while you are away from your home. It lists much of the same vital information as shown above.
